Wikipedia is helpful for current usage.
'Graph' has some more specific technical meanings,
http://en.wikipedia.org/wiki/Graph_%28disambiguation%29

than does 'Chart',
http://en.wikipedia.org/wiki/Chart_%28disambiguation%29

So 'Chart' seems most succinct for the current Activity.
