[Cmacc] master Terms of Use

We participated in the Boston Hackathon over the weekend.  Some good 
materials and relationships from that.

Following up, there is now a fully generalized Terms of Use repository.  
It can be used as a core for any kind of ToU and extended to many other 
kinds of agreements.

ToU is a (close) part of the ecosystem of Consent. They would govern the 
person holding the genome - a critical part of the ecosystem.
