Comment:

 I've found very different USB behavior depending on the position in the
 chain.

 The USB ports directly connected to the system bus are read at boot, those
 on an extension, such as on a monitor or keyboard, are on a hub that is
 not in direct communication with the system at boot time.

 Try connecting to a 'base' USB port, typically on the back of the computer
 next to the motherboard connectors.
